Add Fixed Cash Out Fee

When a member sends a cash out request this addon adds a fixed fee to the requested amount. A fixed fee will be deducted from the requested amount together with the % fee.

Features:

- When a member requests cash out it will save the cashed out amount and the fee (fixed + percent) to the database so when you as the admin pay the member you will send the requested amount minus fee.
- The Fee is added to the admin table so you can see the fee amount and the net requested amount.
- The Decline button in admin table will add the net requested amount + fee back to the member's balance, the member does not lose anything if you decline the payment.
- Fee information added to the member's History so the member can see the fee amount.
